Temp Assistant Care Coordinator (Healthcare) - Up to 8 months #HHL

? Mon–Thu: 8.30am–6pm | Fri: 8.30am–5.30pm

Job scope:
Contact patients about enrolment and follow up with those who have not responded.
Monitor chatbot responses and escalate patient concerns to the clinical team according to protocol.
Coordinate appointments, referrals and handovers between patients and care teams.
Maintain patient records, track service data and prepare reports.

Requirements:
Min. diploma or experience in healthcare administration, care coordination, nursing support or a related field.
Comfortable speaking with patients and using multiple IT systems, spreadsheets and dashboards.
Organised, detail-oriented and able to handle patient information confidentially.
Experience in patient-facing healthcare, telehealth or chronic disease management is an advantage.
