Perm Social Worker (Active Ageing Centre / Community Care) #HZT

Looking to make a meaningful impact in the lives of seniors and their caregivers?

Our client is looking for a Social Worker to join their Active Ageing Centre team, supporting seniors to remain healthy, independent and connected within the community.

In this role, you will work closely with seniors, caregivers, healthcare professionals and community partners to identify needs early, provide psychosocial support and strengthen access to appropriate health and social care services.

Key Responsibilities

• Engage seniors and caregivers to understand their needs, goals, preferences and strengths
• Conduct psychosocial, caregiver and financial assessments
• Develop and review integrated care plans with seniors, caregivers and multidisciplinary partners
• Provide counselling, emotional support and psychosocial interventions
• Support seniors experiencing caregiver stress, grief and loss, social isolation, family issues, dementia-related concerns and mental health challenges
• Conduct outreach, screening and home visits where appropriate
• Develop community initiatives that promote healthy ageing, social participation and connectedness
• Connect seniors to meaningful activities, community resources and appropriate care services
• Provide caregiver education, counselling and support
• Work closely with healthcare professionals, community partners and other service providers to ensure continuity of care
• Maintain proper documentation and contribute to programme evaluation and service improvement

The position also has a strong community development component, including building partnerships, promoting social prescribing and developing initiatives that strengthen resilience and social connectedness among seniors.

Requirements

• Degree or Postgraduate Degree in Social Work recognised by the Singapore Association of Social Workers (SASW)
• Registered Social Worker preferred, or eligible for registration
• Preferably at least 2 years of relevant Social Work experience in community care, eldercare, medical social work, mental health or social service settings will be advantageous
• Experience working with seniors, caregivers or vulnerable populations would be advantageous
• Community development, outreach, group work or community engagement experience would be an added advantage
