Key Responsibilities

• Lead the planning and implementation of Healthy Precinct and community health initiatives.
• Develop community health strategies through data, population health insights, and stakeholder engagement.
• Build and manage partnerships with healthcare providers, community organisations, grassroots leaders, and government agencies.
• Drive cross-sector collaboration to deliver integrated health and social care programmes.
• Lead meetings, monitor project milestones, and evaluate programme outcomes.
• Identify opportunities to improve community health and preventive care through innovative initiatives.

Who We're Looking For
✔ Bachelor's Degree in any discipline
✔ 5–8 years of relevant experience in healthcare sector
✔ Experience in healthcare operations, integrated care, population health, community development, strategic planning, partnerships, or programme management
✔ Strong stakeholder engagement and relationship management skills
✔ Excellent project management, analytical, and communication abilities
✔ Able to work independently in a fast-paced, collaborative environment
