Job Responsibilities
• Support the Inpatient Clinical Operations Team to facilitate seamless transition of patients through proficient social screening and assessment of referrals.
• Assist with bed allocation and bed management tasks to facilitate expedient transfer of patients and ensure optimal utilization
• Perform social and financial assessment of Inpatient referrals including financial counselling to patients and their families.
• Provide the necessary recommendations in the assessments.
• Assist with bed allocation and bed management, when necessary
• Participate in developing and implementing quality improvement initiatives to streamline department operations, to enhance patient outcomes and satisfaction.
• Perform other duties as assigned

Requirements:
• Degree in Social Work or equivalent, preferably with 3-4 years’ social work experience.
• Have an interest in working on eldercare issues at a hospital setting.
• Able to handle administrative role proficiently.
