Location: Upper Thomson
Duration: 1 - 2 months
Salary: $30/hr - $32/ hr

Working days and hours:
- Mon - Fri, 4 hours within office hours

Job Description:
- Develop and implement personalized home care plans based on patients’ needs and conditions.
- Provide basic nursing care, patient education and health coaching.
- Coordinate with doctors, caregivers and community healthcare providers to ensure continuity of care.
- Maintain accurate patient records and conduct regular follow-ups if needed

Job Requirements:
- Valid SNB
- Home care experience is an advantage
