Working Hours: Monday – Friday Office Hours
Salary: Monthly salary based on last drawn and expected salary
Criteria:
• Must be able to read patient’s case note and doctor’s notes
• Must have medication knowledge - able to know why certain drugs are given, what causes the heart failure etc
Major Duties & Responsibilities
• Understand the elements & its definition in the HFSI data collection form and capture those data accurately and timely
• Validate data regularly to ensure the accuracy and completeness in the database
• Understand the heart failure services in the respective institutions
• Monitor, evaluate and communicate relevant programme data to appropriate members of the interdisciplinary team and stakeholders
• Prepare, generate and communicate regular progress reports to the interdisciplinary team and stakeholders promptly and accurately
• Participate in the research and quality improvement activities
• Maintain and update training protocols
• Provide secretarial support in organising conferences and or workshops. This constitutes collating registration for participation, arranging accommodation and purchasing air ticket for overseas speakers, preparing expenditure budget and invitation letter and post conference/ workshop evaluation
• Participate in cluster-wide working committees
• Participate and/or support in departmental level meetings & activities
• Perform other duties as directed by Reporting officer e.g.
o Assist in the formulation and revision of guidelines, standards and procedures
o Participate in research studies for the disciplines or across disciplines
o Participate in quality improvement committees
o Recommend and execute strategies to control LOS and resource utilization across continuum of care
